H1N1 Reporting (SoE Only) - SoE H1N1 Absence Reporting Tool
This article details the SoE H1N1 online absence reporting tool for use by SoE's pandemic coordinator and authorized department contacts.
Background
UW-Madison divisions are required to submit daily reports, Monday through Friday, to campus administration with counts of students, faculty and staff who are out due to flu-like symptoms. The SoE H1N1 Absence Reporting Tool provides a secure, online application for authorized SoE department contacts to submit their counts to SoE's pandemic coordinator for review, collation and submission to central administration.
Access and Use
SoE Intranet Login Screen
- Login at https://intranet.education.wisc.edu
- Use your SOE-AD credentials. For most SoE faculty and staff, these are the same username and password used to login to your office workstation.
SoE Departmental H1N1 Absence Reporting Tool Screen
- Move your mouse cursor to My Applications on the menu bar and select Departmental H1N1 Absence Reporting from the drop-down.
- Type in the number of Faculty/Staff Out
- Press TAB and type in the number of Students Out
- Press TAB and type in any Comments (Optional)
- Click Submit
- Click (Logout)
Notes
- Reports must be submitted daily, Monday through Friday, even if you have zero faculty, staff or students out.
- Each field has an explanatory note. Hover your mouse on the question mark icon to view the related note.
- The date field is pre-populated with the current date. You may not edit this.
- The department field is pre-populated with your department name. You may not edit this.
- The counts for Faculty/Staff and Students should be a cumulative total for the current day.
- Comments may include information about affected courses, student counts tied to particular faculty or instructors, or location information. Comments MUST NOT include the names of the individuals who are out with flu-like symptoms.
- You may ONLY submit your information once daily. The application does not allow you to submit more than once daily, or revise an earlier submission.
- Jo Ann Carr can share report information back for your department, as needed.
