Telephone Troubleshooting and Ordering Procedures

The State Lab of Hygiene occupies two different locations off-campus in the area of Agriculture Drive, one of which require non-standard DoIT Voice Services support and ordering procedures.  This document details those non-standard procedures.  A third site is supported by DoIT using the normal Voice Services procedures.

2601 Agriculture Drive Bldg #8064:  Steve Strebel (224-6216  ss@mail.slh.wisc.edu) is the central telecom contact for the 2601 Agriculture Drive building.  This site is DOA owned and voice services for this location should be handled as outlined below.
2810 Walton Commons West Bldg #8074 is a rental location occupied by the State Lab on the far east side near the 2601 Ag Drive location.  DoIT Field Services supports this site, so any trouble calls or orders for this location should be processed using normal DoIT Voice Services procedures. Dave Miran (221-6282  dem@slh.wisc.edu) is the telecom contact for this site.

Trouble Calls:
The central telecom contact will go through a series of trouble-shooting steps in order to determine the nature of the problem.  These steps should include clearing all variable forwarding on the line by getting dial tone and dialing 118; and unplugging the phone from the wall outlet and replacing it with a working phone. 

Once it is determined that outside help is needed, the central telecom contact should call DoIT Voice Services at 262-5544 option 3 to report the problem.  Voice Services will do additional troubleshooting, and contact DOA if necessary. 

Changing forwarding or standard features on an existing line:
State Lab central telecom contacts should send an e-mail request to voice.services@doit.wisc.edu .

Adding a new line, moving an existing line, adding or deleting a billable feature, adding or deleting voicemail:
DoIT is responsible for providing e911 location information to the PSAP (Public Safety Answering Point) through AT&T for all State Lab of Hygiene Centrex lines, therefore it is imperative for the safety of the State Lab employees that DoIT is involved with all telephone line moves, adds and changes. To place an order, the authorized contact for that UDDS should send an email to voice.services@doit.wisc.edu to request service.  Allow two weeks for orders to complete.

The State Lab of Hygiene should call Sue Streich at 262-0526.  Reference either the UDDS of the telephone numbers, if known, or the AT&T bill number.
