General Student Services Fund Student Served Tracking Form

The Students Served Tracking Form is used throughout the year and is submitted each year to the SSFC Chair.

This form tracks attendance at events and is used by the committee to gain an understanding of growth and accessibility to the UW-Madison student population for General Student Services Funded organizations.

This form is uploaded as part of the End-of-Year Report.
