The UW-Madison Qualtrics Survey Hosting Service allows the campus community to easily create surveys, collect and store data, and produce reports. Users can track email invites and reminder emails, create surveys with the wizard, customize sample surveys for specific projects, and use graphics and statistical tools on the survey results. The website can be accessed online from anywhere. 

The Qualtrics service is available only to UW-Madison faculty, staff, and students. It is for academic teaching or research purposes only and explicitly excludes any commercial use, including commercial and government-sponsored research.
