New Employee Checklist for Managers or Supervisors

The New Employee Checklist is intended for hiring managers or supervisors working directly with recently hired employees. This document contains the basic information Surgery IT needs in order to successfully setup a new user. Please complete the checklist as soon as the employee is hired. This will allow adequate time for Surgery IT to configure the new employee’s IT services and device(s). Additional time will be needed if new equipment must be purchased or shipped out.


New User IT Services Form

The New User IT services form will capture all the IT services a new employee needs access to in order to have a successful first day of work. Managers or supervisors must complete this form as soon as possible.  New User IT Services Form: 

NetID Activation:

The hiring manager or supervisor will notify the new employee to activate their NetID. If the employee is not hired by SMPH, Surgery IT will notify the new employee to activate their NetID by email. All new employees should activate their NetID’s at least 3 days before their start day. This ensures all core campus services like Duo and Outlook email are working properly.

NOTE: ALL SMPH employees automatically receive a NetID. Internal department services like Network drives or Quicklinks access only require a Surgery account, which is created at the time the New User IT Service Form is completed. A NetID is only required if the new user will need access to UW-Madison services like Outlook calendar, WiscVPN GlobalProtect access, or a Department of Surgery email alias and are NOT employed by SMPH. 

Hardware Request Qualtrics Survey Form: (if working remotely/hybrid) 

If the new employee is working remotely/hybrid, hiring manager or supervisor should notify the user to complete the Hardware Request Qualtrics Survey form as soon as they are hired. This allows IT enough time to order and ship out equipment to the user before their start date. It is expected that the new employee will request hardware that has already been discussed and approved by the hiring manager or supervisor. Hardware Request Form Link: 

Please note all new remote employees should have the Remote User Agreement approved by the hiring manager or supervisor before submitting a hardware request. 

IT Orientation

Hiring mangers or supervisor are responsible for scheduling an orientation date and time with Surgery IT before the new employee start date. This will be done by emailing  and requesting an orientation. We may not be able to accommodate last minute requests (ex: on the new employee start date).
