Student Center - Updating Your Legal Name, Birthdate or Gender

An overview and step-by-step guide to updating your personal information in your UW-Madison student records: legal name, legal gender and birthdate.

Overview:

Once a request has been submitted, please allow 3-5 business days for it to be reviewed and processed by the Office of the Registrar.
You will receive an email notifying you if the request has been approved or denied (in the latter case you will also be provided with the reason the request was denied).
Note: Requests sent by international students with an F-1 or J-1 visa will be routed to International Student Services before they can be reviewed by the Office of the Registrar. Please allow longer than 3-5 business days for the request to be processed.

Employees (including student employees): please note that you will also need to initiate a change to your personal information through your employing department.

Applicants: Please contact the Office of Admissions with your request as you will not have access to the online form referenced in this document.


Update Your Legal Name

  1. Login to MyUW.
  2. Click on the Student Center tile, then the Profile tile.

    Alternatively, you may select the Student eForms tile and processed to click on Personal Information, then Personal Info Change Request. If you have selected this route, skip step 3.

  3. From the menu on the left, select Personal Info Change Request.

  4. Review the personal information currently on your student record. If the information under "Name" needs to be edited, select "Legal Name Change" from the Select Desired Change drop-down menu.

  5. Your current name on record will be auto-populated. Make any necessary changes and scroll down the page.
    Note: First and last name are required fields. If your legal name includes only one of these fields, please enter . (a dot) in the empty field.

  6. Click the Upload button to add an attachment. The attached document should show your corrected/updated legal name (e.g. Driver license, passport, court document, or birth certificate). International students with an F-1 or J-1 visa must upload their passport for documentation.

  7. A pop-up window will display. You can select a file from your computer by clicking "My Device" and browsing your computer, or drag and drop any file into the area with a dashed border.
    Once the file loaded, click "Upload", then "Done".

  8. Enter an appropriate description of the document uploaded.
    Click "View" to preview the document or "Replace" if needed. If you need to add more than one document, click "Add" to create a new row for an attachment and repeat the same steps to upload a document.
    Once all documents have been uploaded, scroll down the page.

  9. Review the acknowledgment and click the toggle button to approve.

  10. If you would like to add any comments to your request, click "Comments" and add the text. When you are done, click the Submit button to complete your request.

  11. Your request has been submitted and sent to the Office of the Registrar for review. Please allow 3-5 business days for the request to be reviewed and processed.

Update Your Birthdate

  1. Login to MyUW.
  2. Click on the Student Center tile, then the Profile tile.

    Alternatively, you may select the Student eForms tile and processed to click on Personal Information, then Personal Info Change Request. If you have selected this route, skip step 3.

  3. From the menu on the left, select Personal Info Change Request.

  4. Review the personal information currently on your student record. If the information under "Date of Birth" needs to be edited, select "Birthdate Change" from the Select Desired Change drop-down menu.

  5. Your current date of birth on record will be auto-populated. Make any necessary changes and scroll down the page.
