Putting in a Request for Prism Graphpad

Instructions for how to put in a request for Prism Graphpad and its activation

Currently Graphpad licenses can be requested through this simplified google form: https://forms.gle/RyoE1cdicEL2W7Bx8
- Note: You must be signed into your UW account in order to access the form
License request are then processed through POS for the transaction
Email is then sent to the user from Graphpad with activation information
