Acuity One45 - How to Log In (UW-Madison and partner campuses)
Describes how to log into the Acuity One45 system for individuals who are on the UW-Madison campus and on the extended campuses around the state of Wisconsin.
UW-Madison Campus Log In (UW-Madison Net ID)
Members of the UW-Madison campus and those who hold UW-Madison NetIDs may log into Acuity One45 using their UW-Madison NetID and password.
- Navigate to https://uwsmph.one45.com/.
- Click Login Here on the left side of the menu.
- After clicking Login here, the screen will redirect to the UW-Madison Net ID login page.
- Enter your UW-Madison Net ID and password in addition to your dual authentication.
- After successfully completing step 5, you will be logged into your Acuity One45 account.
- The automatic landing page for Acuity One45 is your To-Do page.

Extended Campus Partners (No UW-Madison Net ID)
Users without a UW-Madison Net ID (Never Logged into Acuity One45 Before)
Extended campus partners and individuals who do not have a UW-Madison net ID, may log into Acuity One45 using their preferred email. **These instructions are for users who have never logged into Acuity One45 before.**
- Navigate to https://uwsmph.one45.com/.
- Click Login button on the right side of the menu.
- Next you will be prompted to enter your username in password. Do not enter this information and instead click the link Need help logging in?
- The screen will refresh and you will be asked to enter your email address. Then, click Send email reminder.
- Clicking Send email reminder, will send a password reset message to the email address you have entered. Example below:
- The body of the email will contain your username and a link to your password reset page. Click on the link. Please note that by default, the link is valid in your email for 10 minutes and can be clicked on multiple times during that 10 minute period. But it will expire right after the password is reset.
- From there you will be asked to create a new password.
- One45 password policy: Minimum 8 characters, more than 3 unique characters.
- After clicking Submit, you will be logged into Acuity One45 and taken to your eDossier within the system.
Users without a UW-Madison Net ID (Logged into Acuity One45 Before)
Extended campus partners and individuals who do not have a UW-Madison net ID, may log into Acuity One45 using their preferred email. **These instructions are for individuals who have previously logged into the Acuity One45 system.**
- Navigate to https://uwsmph.one45.com/.
- Click Login button on the right side of the menu
- Next you will be prompted to enter your username in password. Once enter, click the login button.
- After clicking login, you will be logged into your Acuity One45 account and taken to your eDossier within the system.
