Acuity One45 - How to Reset your Password (Individuals without a UW-Madison Net ID)

Describes how to reset the login password for Acuity One45. The password reset applies to only individuals who do not have a UW-Madison Net ID and password.

**Individuals with a UW-Madison Net ID with be logged into Acuity One45 with their UW-Madison Net ID and password. Please refer to NetID - Changing a Password for instructions on how to change your NetID password**. 

For Acuity One45 users who do not have an UW-Madison Net ID, please use the instructions below on how to rest your Acuity One45 password. 

  1. Navigate to https://uwsmph.one45.com/. 
  2. Click Login button on the right side of the menu.

  3. Next you will be prompted to enter your username in password. Do not enter this information and instead click the link Need help logging in?
  4. The screen will refresh and you will be asked to enter your email address. Then, click Send email reminder.

  5. Clicking Send email reminder, will send a password reset message to the email address you have entered. This is an example of what the email will look like.

  6. The body of the email will contain your username and a link to your password reset page. Click on the link. Please note that by default, the link is valid in your email for 10 minutes and can be clicked on multiple times during that 10 minute period. But it will expire right after the password is reset. 
  7. From there you will be asked to create a new password.
    1. One45 password policy: Minimum 8 characters, more than 3 unique characters.
  8. After clicking Submit, you will be logged into Acuity One45 and taken to your eDossier within the system.
