Guide to Purchasing Items under $5000
Start here:
- Is this purchase necessary, reasonable, and allowable?
- Did you check if the item is available on Shop UW+?
- If it is, order through Shop UW+. See HERE for information on how to use ShopUW+
- Please fill out the Purchasing Request Form
- This form is routed directly to the purchasing department. Note: CHM Purchasing (chm_purchasing@chm.wisc.edu) will send you an email to confirm your request was received, along with a copy.

- Fill out the Department Card Checkout Form if you will be making the purchase directly.

- Please contact this email for all purchase-related communication. It ensures efficient processing by our purchasing team.
Tax Exempt
- We cannot pay tax - remember to show tax exempt information to remove sales tax
Business Meals / Hosted Events
Printing
- If a printing job costs over $50 it MUST be ordered through Digital Printing & Publishing Services, or University Marketing.
- Use campus resources whenever possible for under $50 jobs
Software
- First, check UW Campus Software Library to see if a contracted vendor can meet your needs
- If not, then fill out this Risk Review Intake form
- Any software must go through the Cybersecurity Review process and be approved BEFORE making the purchase (please allow plenty of time for this, usually ~1 month, sometimes more)
