Facilities: Intake Form
The intake form communicates essential information to the School of Music's Technical, Operations, and Communications teams. It must be submitted in order to finalize the reservation process for all events at the Hamel Music Center that require technical/production support or are open to the public. Your reservation request will not be confirmed unless this form is submitted in a timely manner. Please complete this form after reserving a space in EMS and paying any applicable fees in the Online Storefront.
