Assigning Clinical Performance Assessments

Use this document for help assigning the correct forms to be used in different clinical settings, both by block and by department. For help with setting the assessments up in Acuity One45, use the Phase 2 Evaluation/Assessment Overview and Process KB.

Assigning Clinical Performance Assessments By Block

Acute Care

  • Internal Medicine teams – Inpatient
  • Emergency Medicine supervisors – Inpatient
  • Neurology teams – Inpatient
  • Psychiatry teams – Inpatient
  • Small Group Leader – CBL

Specialized Care of OB/GYN and Pediatric Patients

  • Pediatric inpatient team – Inpatient
  • Pediatric ambulatory clinics* – Outpatient
  • OB/GYN Antepartum service – Procedural
  • Labor & Delivery – Procedural
  • Surgical Gynecology – Procedural
  • OB/GYN Ambulatory clinics* – Outpatient

*includes longitudinal preceptor assignments

Chronic & Preventive Care

  • Clinical preceptors – Outpatient
  • Small Group Leader – CBL

Surgical & Procedural Care

Preferred Clinical Assessment Parameters

See the preferred clinical assessment parameters to use when setting these up in the evaluation workflow in Acuity, as given by the Clinical Assessment Task Force.

  1. Form: Appropriate clinical assessment form determined by the above matrix above
  2. How many days before the end of the block should this form be sent?: Department preference
  3. Evaluator -> Target: Faculty->Student
  4. Use this form for which students?: Both (internals and externals)
  5. Which faculty will evaluate each student?: One Form Distributor will decide for all students (enter self as distributor)
  6. Use schedule matching if available: Yes (check the box)
  7. Is there a Summary or Head Evaluator with this form?: No
  8. Are the results of this form anonymous? No
  9. Do you want the target (student) to see the results of this form in their inbox?: Yes, as soon as the results are available
  10. Can the evaluator (faculty) choose to opt out of completing this form?: No
  11. How many days should this form be available before being canceled?: Department preference
