The Independent Learning Waiver must be emailed to SOHE Deans Services deansservices@sohe.wisc.edu by the 5th Friday of the fall/spring semester. Students will be contacted once the form is signed and can be picked up at the Academic & Career Center in Nancy Nicholas Hall. Students are responsible for submitting the form to UW Extensions.
