Transferology - Submit the Add an Equivalent to Transferology Request Form
Overview
Credit Evaluation Services has created a form through which current undergraduate students can request that we add an equivalent to Transferology. It is available via an eForm in a tile in your Student Center in MyUW. Putting it within your Student Center allows us to tie it directly to you without you needing to fill in your name, campus ID, and contact information. Please follow the instructions below. Filling out the form correctly is important to making sure your request is able to be processed as quickly as possible.
Before Getting Started
To request a course be evaluated to determine whether an equivalent UW–Madison course exists:
- You must be a currently enrolled UW–Madison undergraduate student. Special students and prospective students are not eligible to use this service.
- The course you are requesting we review must be from a U.S. institution and in that institution's course catalog. Courses from non-approved study abroad programs or international institutions cannot be equated before the course is taken. Courses not in the regular course catalog will not be visible on Transferology and are not eligible for pre-review.
Each form submission can be used for only one course. While you may submit more than one form, this service is meant to be used for courses a student truly intends to take. Submitting a large number of requests for courses you are not really intending to take just to see how they transfer will result in delayed processing time for everyone using the service. The CES team reserves the right to deny excessive requests.
What You'll Need
Before you begin your request, make sure you have the following information:
- A syllabus for the course from the past academic year that includes (at a minimum) information about the textbook used, the assignments/exams required, and a weekly schedule of topics covered. If the course contains a lab, the syllabus must also include a schedule of experiments performed;
- The name of the institution offering the course;
- The abbreviation for the department offering the course, as listed on Transferology (i.e. "POLI SCI," "CHEM," "MAT")
- The course number/title; and
- The number of credits.
Steps
- Log in to your Student Center. Select the Student eForms Tile.
- In the UW Student eForms Center, click into the Student Services folder on the left, and select the form called Add to Transferology.

You should see a section with some instructions, and then a Student Information section that has pulled in your information. Scroll down to the next section – Course Information.

The Course Information section has the following fields for you to fill out:

School Name: This should be the name of the school where you intend to take the course.

Department: Enter the department abbreviation as it appears on Transferology. This field is limited to 10 characters, so if the abbreviation is longer, just type the first 10 characters.

Corresponding UW–Madison Department: This is the UW–Madison department you think the course would belong in.

Click the magnifying glass to open a search window.

Departments are listed alphabetically, or you can click the Search Criteria drop-down to search for a specific department. The UW Subject Department field is our department's abbreviation, so if you don't know it, just search on the first letter.

Type the first few department letters in the UW Subject Department field and click the search button.

Click on the result you want and it will enter it into the form field.

NOTE: If there is no clear matching department, search for "UNKNOWN" OR "UNK" and use that option.
It’s very important to try and put either a matching department or “UNKNOWN” as this speeds processing time. If you just select the first department you see, for example, you may slow down the processing of your form as it will be incorrectly routed and will take time to discover and re-route.

Course Number: This is the course number of the course you plan to take as listed on Transferology.

Course Title: As much of the course title as you can fit. The field is set to 75 characters – it’s okay if it cuts you off at some point.

Credits: The number of credits the course is offered for at the other institution. You will earn the same number of credits you receive. If the school is not on a semester system, we will convert this value to semester credits once you’ve submitted your transcript and we are ready to transfer the credit.

Upload your syllabus
The syllabus must be a .pdf or Word document of less than 10MB. It must be from the past year, and contain at a minimum textbook information, a course schedule (including lab schedule if labs are part of the course), and the assignments and exams required for the course. If you have more than one document, you can upload up to two. If you have more than two documents, you will need to combine some before upload.

Click the upload button.

In the File Attachment window that opens, click which device you are selecting from.

Click Open once the document is selected.

Click Upload on the File Attachment window.

Click Done to exit the File Attachment window once the file has been uploaded.

Click Add to add a second attachment if necessary.

Read and then complete the acknowledgment by toggling it to Yes.

Click the Submit button.

Confirm that you have received the successful submission screen. You will be able to check Transferology within 4-6 weeks for your equivalent.
If you wish to submit another form immediately, simply click the tab with the two parallel lines at the left side of the page to return the Student Services menu and click the Add to Transferology form name again.
Notes
- All student-submitted syllabi are subject to verification by the external institution, must represent the actual coursework and learning outcomes completed at the external institution, and may not be modified, altered, or revised by the student. Students who submit course syllabi that are forged or falsified may be investigated for and subject to sanctions under UW–Madison's Academic Misconduct policy.
