Apple Device Passcode Reset or Activation Lock (Cellular-Mobile)

Forgotten or Unknown PIN/Passcode, iPhone Locked

Reset PIN/PASSCODE on device – PASSCODE Lock:

This will require a computer and some software being downloaded to the computer and the phone to be in recovery mode (times out after 15 minutes).

Activation Lock – Phone activated with an Apple ID and/or has Find My Phone activated

The AppleCare agent you speak with may ask you to provide the following information via a secure portal they will generate for you on a case by case basis:

  • Institution name:  UW Madison DoIT
  • Device serial number(s) and/or IMEI/MEID
  • Proof of Purchase (**request from DoIT Via HD Ticket  For other questions related to Cellular service, please use this form**)
