Blue - Question Personalization (QP) (Instructor) [UW-Madison]

In this document you’ll learn how to use question personalization (QP) for your course evaluation.

Instructors can add two additional questions to their student evaluations of instruction for course sections they teach. These questions are known as Question Personalizations (QP). All other questions in the evaluation are preset and cannot be changed.

Cross-listed Courses: In cross-listed courses, instructors must add QP for each section for all students to see them.

Question Personalization (QP)

  1. You will receive an email when you can begin QP.
  2. In the email either open the link or in your web browser, navigate to https://studenteval.wisc.edu 
    Note: Alternatively, open your Canvas course page and select Course Evaluations from the sidebar.
  3. Log in with your NetID and password
  4. Blue will list all course sections that have open evaluations. It will also say when they are due. If a course section is eligible for QP, it will say Start Now.

  5. Select Start Now beside the course evaluation you would like to modify. This will open the evaluation.
  6. To see the questions that are already in the survey, choose Preview.

  7. Blue displays the message: by selecting “Consent,” you acknowledge that you understand your responses may not be anonymous in this small-enrollment course and agree to participate in this evaluation. Please choose “Consent” or “Decline” to continue.

  8. You will need to select Consent to continue. This message displays for all previews, but will actually only display on the evaluation if a course has fewer than 5 students enrolled.
  9. Instructors may add two open ended questions. To add a question, select Customize.

  10. Once you add your question, click the x to exit. There is no save button at this time.

  11. Finally, select the submit button to submit your question(s).

Note: Remember, you may add up to two questions. The questions you add will only be part of your reports; not part of the department and/or college’s reports.
