Acuity One45 - Direct Observation Forms

This document explains how students can initiate thier own direct observation forms in Acuity One45.

Sending Direct Observation Forms

Students are able to initiate direct observation forms to specific supervisors for Phase 2 within Acuity One45. The option to initiate these forms will be available on the students' To Do page throughout the course so that students may send them at any time. These forms will be available the first day of a block and 30 days after its end. 

  1. Sign in to Acuity One45: https://uwsmph.one45.com/web/one45.php/
  2. The default landing page is the To Do page

  3. Find the header Forms to Send

  4. Click on the green plus button Green Plus Button or on the form title. Note: There are two separate direct observation forms. Make sure you select the correct form.2
  5. A new window will open to enter information about the evaluation.
    1. Step 1: Select an Event: Two options will appear. The "attending" option will include all faculty. The "resident" option will include residents and teachers (non-faculty preceptors)

    2. Step 2: Select a Date: Enter the date in which you are initiating the evaluation (i.e. today's date)

    3. Step 3: Choose Who Will Be Assessing You: Select the supervisor or multiple supervisors from the list.

  6. Clicking Send Form will send the form(s) to the supervisors' To Do page which they may complete. 
  7. The default deadline for self-initiated forms is two weeks. A deadline will appear after the faculty name in the To Do list.
