Privacy and Confidentiality
Question 1.1: Describe how participants privacy will be protected. For example, conducting research intervention in
a private room, only collecting sensitive information about subjects that is pertinent to the research question, etc.
Question 1.2: Describe how participants are identified in the data/records throughout the study:
- Directly: Research data/records contain direct identifiers such as participant name, or other identifying information
- Indirectly: Researchers have or collect participant identifiers, but store the identifying information separately from research data/records
- Anonymous: No identifying information is collected from participants; very few research projects have no identifiers (meaning that the participants are truly anonymous)
Question 1.3: Describe how research documents and data will be stored and protected, to protect against a breach of confidentiality, etc. (ensure this description complies with campus policy*)
Question 1.4: If data will be retained for other research or uses beyond this study, information should be included in the consent form to notify participants.
