Pharmacy No. This field is used to capture the sponsor-assigned protocol site number for the UWCCC because UW Health Pharmaceutical Research Center (PRC) does not assign a unique number to protocols.

Required in the situation described

Oncology: Required if the sponsor has assigned a unique protocol-specific site code to UW.

Site codes for NCI studies are not required to be entered into OnCore because they are site-specific, rather than study-specific. The site code for UW Hospital is WI020. The UW site code for Big Ten Cancer Research Consortium is 363.

Non-oncology: Optional
