CSD/UWSHC Fax Number
CSD and UWSHC has (608) 262-6466 setup for incoming faxes. Once a fax comes into the system, the recipient will be forwarded an email message with a pdf attachment. Please contact Michele Myers and Sherie Felicijan as they routinely monitor the incoming fax number/email.
