How to complete the Systems Engineering New User Request Form.

File Server Access Request Process

File Server Access Requests are typically sent to the Office of Cybersecurity ERP teams via JIRA. Requests may come in through other means, however a JIRA should be created to document the request, and gather any necessary approvals. Access requests will involve the following individuals:

  • User: The person receiving the File Server access.
  • Sponsor: Typically the user's supervisor but could be the JIRA requester/reporter.
  • Requester/Reporter: The individual who contacted Cybersecurity with the request. Must be an employee of UW-System Central Support, the UW Service Center, or DoIT.
  • Security: The individual submitting the Systems Engineering New User Request Form.

First, check the 'members.txt' files located on the Holmstrom shared drive. (//holmstrom.ad.doit.wisc.edu/security/)

  1. Check the HRS, ImageNow, SIS, or SFS folder of the corresponding ERP for the request.
  2. If user has no existing access, fill out the Systems Engineering New User Request Form. Systems Engineering will only accept new user requests from Office of Cybersecurity team members.
  3. If user has existing access, but needs additional access, use the existing account information from the 'members.txt' file to complete the Systems Engineering New User Request Form specifying the additional access that is needed. 


Completing the Systems Engineering New User Request Form:

The following information is required and should be included on the JIRA:

User Account Information

  1. First Name
  2. Last Name
  3. Phone Number
  4. Email Address
  5. DoIT Group
Sponsor Account Information (typically the user's supervisor or the JIRA requester/reporter)
  1. First Name
  2. Last Name
  3. Phone Number
  4. Email Address
  5. DoIT Group
Access Details
  1. Hostname or Server name, such as:
    1. sfsclient.ad.doit.wisc.edu
    2. hrsclient.ad.doit.wisc.edu
    3. sfsproc1.ad.doit.wisc.edu
    4. \\typhoon\
  2. Operating System: Windows
  3. Requested User Name: User’s NetID
    • For SFS, if no NetID, use firstinitiallastname_sfs (ie. Travis Brown = tbrown_sfs) – all lowercase
  4. Account Type, Replicate Existing Account, Shell: Accept the Defaults
  5. Account Needed by: Give Windows Team 1 Week
  6. Account Expiration: Never (Default)
  7. Account Used For: This explanation should be listed on the JIRA
  8. Account Security: Accept Defaults
  9. Special Information: Include File Server Role information & if user does not have NetID
SE New User Form

Click the Submit Button. Email notifications from the
Systems Engineering Windows Team will be send to the User, the Account Sponsor, and you as the form submitter.

Once the Windows Team responds add notes to the JIRA, and send JIRA back to the Requester/Reporter for verification and closure.
