Risk Management - Injury & Incident Reporting - FAQs
FAQs
Who needs to have an emergency phone number listed in the directory for the phone tree?
Extension supervisors and leadership positions.
Who can access the emergency phone numbers in the directory?
They are password protected in the internal directory. Extension employees and POIs with a UW-Madison NetID will have access.
Am I required to list my personal cell phone number?
No. Please list a work cell phone if you have one. Otherwise you can list a work landline.
What happens if I am not able to answer when someone initiates the phone tree?
They should continue calling up the phone tree depending on the high-risk injury/incident. You are not responsible for monitoring your phone 24/7. Risk management reporting time frames also offer some flexibility (24-48 hours for submission). We understand that employees may be out of the office or unavailable for various reasons and circumstances.
Am I expected to be on call 24/7 if I list my work cell phone or personal number?
No. This phone tree is an escalation tool. We understand that there may be some limitations due to a variety of reasons and circumstances.
