How to complete the general funding page, the funded studies page and the funding information page

Funding: General

What is the purpose of this page?

The Funding: General page of the General Study Information section allows you to indicate the type of appointment under which the Principal Investigator (PI) will conduct the study. It also asks you to indicate whether the study is funded and, if so, the entity that will administer the funding.

When does the Funding: General page display?

The Funding: General page displays for all types of studies and is page 7 of the General Study Information section. Based on the answers provided on this page, the study team may be branched to other pages of the smartform.

How should question 7.1 be answered?

Select the organization through which the PI will conduct the study. Only 1 option may be selected. NOTE: If the PI has an appointment with both the UW-Madison and the Madison VA, you will need to determine the appointment under which the PI will conduct the specific study.

How should question 7.1.1 be answered?

Question 7.1.1.1 needs to be answered if the PI will be conducting the study through the University of Wisconsin-Madison (as indicated in question 7.1). Click the Select button in question 7.1.1 and choose a department from the list provided. If the PI has appointments in more than one department, choose the department for which the PI holds an appointment that allows him or her to engage in research activities. Do NOT select a program or center (e.g., Cancer Center) unless it represents the PI’s home department.

How should question 7.2 be answered?

Indicate whether the study will or plans to receive funding to support the study or project. If the study is not or will not be funded, answer "No" and no additional questions about funding will need to be answered. If the study is or will be funded, answer "Yes"

How should question 7.2.1 be answered?

If you answered "Yes" in response to question 7.2, you will need to answer 7.2.1. For studies that the PI will conduct through the University of Wisconsin-Madison (as indicated in question 7.1), please follow these guidelines for answering question 7.2.1:

  • Answer "Yes" to 7.2.1 if the study is funded and the account must set up through Research and Sponsored Programs (RSP). You will then be directed to other pages of the form that will request additional information.
  • Answer "No" to 7.2.1 if the study is funded by the Institute for Clinical & Translational Research (ICTR) grants, WI Partnership/MERC/OAC, or UWCCC Investigator-Initiated trial (IIT) grants. A copy of the notice of award should then be uploaded in the Supplemental Information section (question 2.1) located at the end of the initial review smartform.

How should question 7.2.2 be answered?

If you answered "Yes" in response to question 7.2, you will need to answer 7.2.2.

  • Answer "No" to this question if the PI will conduct the study through the University of Wisconsin-Madison.
  • Answer "Yes" to this question if the PI will conduct the study through the UWHC or Madison VA AND the UWHC or Madison VA will administer the funding for this study. You will then be directed to other pages of the form that will request additional information.


Funded Studies

What is the purpose of this page?

The Funded Studies page in the General Study Information section allows you to choose all sources of support for the research study. Based on the options selected on this page, the study team may be branched to additional pages of the smartform to answer questions about a particular type of funding.

When does the Funded Studies page display?

The Funded Studies page is page 8 of the General Study Information section. This page will display if the study team indicates on page 7 of the General Study Information section (question 7.2) that the study has or will receive funding.

How do I know which funding source to choose?

The Funded Studies page (question 8.1) lists the types of funding sources the IRB most frequently sees. Select ALL the funding types that apply to your study.

What if the type of funding that my study will receive is not listed in question 8.1?

If you do not find your funding source among the list of funding types provided, choose the "Other" option and then describe the nature of this funding in response to question 8.1.1 on this page.

Where do I provide additional information about the study’s funding?

  • If Private foundation or association (non-industry) is selected, the Funding: Private Foundation or Association (Non-Industry) page (page 11) also will need to be completed. You will be asked to provide the sponsor’s name and address questions related to the sponsor’s policy related to IRB fees.
  • If Industry sponsored is selected, the Funding: Industry page (page 12) also will need to be completed. You will be asked to provide the name of the sponsor, indicate whether the sponsor has a limited role (i.e., only provides the drug or device and no further support), and what the expected F&A rate is.
  • If Institute for Clinical & Translational Research (ICTR) grants, WI Partnership/MERC/OAC, or UWCCC Investigator-Initiated trial (IIT) grants is selected and the study will be conducted through the University of Wisconsin-Madison, additional information about the funding source will be provided in the Supplemental Information section at the end of the initial review smartform.
  • If other types of funding are selected (e.g., Core grant, Research funding from a federal agency, VA research grant, etc.), additional information about the funding source may need to be provided on the Funding Information for Study Conducted under Madison VA (Wm. S. Middleton VA Hospital) or UWHC page (page 9) OR the Funding Information for Study Conducted Under UW-Madison page (page 10).


Funding Information Pages

What is the purpose of these pages?

The Funding Information for Studies Conducted under the Madison VA (William S. Middleton VA Hospital) or UWHC page (page 9) and the Funding Information for Studies Conducted under UW-Madison page (page 10) allow the study team to provide additional information about funding sources.

When do these pages display?

The funding information page for the Madison VA or UWHC (page 9) displays if the study team answer "Yes" to question 7.2.2 on the Funding: General Page. The funding information page for UW-Madison (page 10) displays if the study team answer "Yes" to question 7.2.1 on the Funding: General Page.

How should question 9.1 be answered?

  • Click the Add button in question 9.1 to open a pop-up window that will allow you to enter information about each funding source.
  • If you have multiple funding sources, enter the information for each funding source one at a time.  Use the Add button as many times as needed to enter all funding sources.
  • If the study is supported bu a federal grant and the Madison VA or UWHC holds the grant, you will also need to upload a copy of the grant in the section.

How should question 10.1 be answered?

  • Click the Add button to choose your source of funding from a list generated from databases maintained by RSP (WISPER/WISDM) in question 1.  You can search this database in several ways, including by PI name, study title, or project ID number.  In question 2, you will be asked to upload your grant application.  NOTE: Your proposal must be submitted to RSP before submitting your application to the IRB.
  • If you have multiple funding sources, enter the information for each funding source one at a time.  Use the Add button as many times as needed to enter all funding sources. Funding question popup window

How should question 10.2 be answered?
  • Study teams must provide a backup account number UNLESS: 1) the research is being conducted by a PI under his or her appointment outside the School of Medicine and Public Health; 2) the research is federally supported and the funds have been awarded; OR 3) you are submitting an exemption, protocol development activities (PDA), emergency or one-time use, or humanitarian use device application.

How should question 10.3 be answered?

  • If your department does not have a designated accounting point of contact, please list the person who should receive all funding related questions for the study.
