Death of Employee/Dependent

Please refer to the steps below when you are aware of a death of an employee or a dependent of an employee.

In the event of the death of an employee, please do the following:

1. Email Madison Benefits Services (MBS) at benefits@ohr.wisc.edu and copy your HR/Payroll Representative in our office as soon as you are able, even if the employee was not enrolled in benefits. MBS needs to complete several processes related to the employee’s death.

Please provide the employee’s name, Employee ID, Employee Record and the date of death.

 

2a. Decentralized Departments and Centralized with Delegation Departments
Make an entry in JEMS to terminate the job so we can pay the deceased employee correctly. The effective date of the termination would be the day after the death.


2b. Centralized Departments
Complete the L&S Job Change Form found at https://kb.wisc.edu/ls/58371 so we can pay the deceased employee correctly. The effective date of the termination would be the day after the death.
