HelioCampus AC - Eval Survey Form - Deleting a Survey Form (Admin) [UW-Madison]
This document describes how to delete a survey form in HelioCampus AC. Only HelioCampus AC administrators can modify survey forms.
HelioCampus AC survey forms can be permanently deleted only if they have never been used to survey students. Survey forms that have previously been used cannot be deleted. Please note: In the latter case, the "Delete" command is still available to the HelioCampus AC departmental administrator. However, instead of actually deleting the previously used survey form, HelioCampus AC will only mark the form as "inactive".
Navigating to Survey Form Manager
- Log into HelioCampus AC with your NetID credentials.
- Select the Menu button (three stacked lines) on the left side of the page to open the HelioCampus AC main menu.
- Select Survey Form Designer under Surveys.
Deleting a Survey Form
- After navigating to Survey Form Manager, find the survey form you wish to delete and select Manage.
- Click Actions and select Delete Template.
Note: On this menu, your survey form is referred to as a Template. This is how survey forms were labeled before AEFIS changed to HelioCampus AC. - You will be asked whether you wish to continue. Click Delete Survey Template.
Accessing Inactive Surveys
If a survey form has been used to survey students, the deletion process above will simply deactivate the survey. Inactive surveys can still be accessed by HelioCampus AC administrators.
- Navigate to Survey Form Manager.
- Click + Add Filter and select Status.
- Select Inactive.
