HelioCampus AC - Eval Survey Form - Using the Rollup Questions Placeholder (Admin) [UW-Madison]

When creating a survey form, HelioCampus AC administrators are encouraged to include a question of the type Rollup Questions Placeholder. The Rollup Questions Placeholder allows administrators and instructors to add course section specific survey questions to a survey after the form has been published.

Adding a Rollup Questions Placeholder to a Survey Form

Create a new survey form, or navigate to an existing survey form.

  1. To create a new form, refer to HelioCampus AC - Eval Survey Form - Creating a Survey Form (Admin) [UW-Madison].
  2. To edit an existing form, refer to HelioCampus AC - Eval Survey Form - Editing Title and Details (Admin) [UW-Madison].
  3. The survey form must be in draft mode to make changes. If it has already been published, you must unpublish it. If it has already been used by a survey schedule, then you cannot unpublish it. Instead, copy this survey form, and create a new one.
  4. Add questions to your Survey Form, following the normal process. See HelioCampus AC - Eval Survey Form - Creating and Editing Questions (Admin) [UW-Madison].
  5. We recommend the last question you add be a Rollup Questions Placeholder. Enter descriptive text if desired and save.
    Rollup Questions Placeholder is an option in the left panel of the Add Question pop-up
