The Entertainment Contract is used to contract services from outside performers for entertainment purposes. Click here to download a copy of the contract.

Along with the contract, you must submit a W-9 for the performer.

This contract must be submitted to the GSSF Program Specialist at least 2 weeks in advance when necessary for event purposes.
