SMPH HSLC Network - New Employee Account
Steps required for a new employee to access SMPH/HSLC computing resources.
The request for a UserID on the local SMPH/HSLC network, should be preceded by obtaining a UW ID and a NetID. New hires are assigned a 10-digit UW ID number at the time of hire.
The UW ID number can be obtained in advance of start date, by following the instructions at:

Once the UW ID number has been obtained, the new employee can create a NetID account here:

To apply for a UserID on the SMPH/HSLC network, the employee's supervisor (or other SMPH contact) must complete the web form at:

Your network account will implement a username, that is the same as your UW NetID username. A temporary password will be assigned, which the new user must change at first login. We also require a valid "wisc.edu" email address, to obtain a user account on the SMPH network. The supervisor should indicate specific departmental shared folder(s) to which the new user should have access.
The supervisor will be notified when the new user account is created, and the temporary password has been assigned. The new employee will be prompted to set his password at first logon (for Windows computers) or can submit to the following web form:

The notification email to the supervisor will also include the outline of network policies and procedures which should be forwarded to the new employee.

One-on-one orientation of the new employee, by SMPH CIT Desktop Support staff, can be scheduled at the new employee's convenience, by opening a case with the DoIT Help Desk (specify HSLC Gold and the supervisor's name).
