MPH Background Check and Drug Screen Procedure
Students in the Master of Public Health Studies (MPH) will occasionally need to complete urine drug screen (UDS) and background checks to comply with site requirements for their Applied Practice Experience (APEX). When these screenings are required for a student, the process below will be followed.
Process

MPH staff will email the Academic Affairs Compliance Specialist (ckforrester@wisc.edu) with the request, including the student’s name and Wisc email address.

Compliance Specialist will initiate the required screenings through the Universal portal using the e-Forms Invitation feature.

Student will receive email directly from Universal to complete and pay for their required screenings.

If a urine drug screen (UDS) is required, student will need to provide sample to the lab approved by Universal.

Students will receive access to their background check and UDS results once complete.

Students are responsible for providing results to their APEX site.
