Multi-Site: UW-Madison Serving as IRB of Record
Question 2.1: All the non-UW Madison sites and/or personnel involved in the study should be listed here.
Question 2.2: If you are requesting that the ED/SBS IRB serve as the IRB of record for a non-UW-Madison individual or entity, select either:
- Individual affiliated with an institution with an IRB NOT listed above
This would be a collaborator who is affiliated with an institution (such as another university) which has their own IRB. In this case, the IRB will need evidence that the collaborator's home institution does not consider the institution engaged in the research, and that their IRB review is not required. This means that the collaborator's home institution will cede IRB review to UW-Madison, who will consider that person the same as any other member of the study team who is affiliated with UW-Madison. The IRB will require this collaborator take either UW-Madison human subjects training through CITI or comparable human subjects research training elsewhere.
- Individual NOT affiliated with any institution or with an institution with NO IRB (e.g., community organization or hospital)
This would be a collaborator who is not affiliated with any institution with their own IRB. This would likely be someone who will be engaged in research activities at a community site, which does not have an IRB or other review process. The IRB will require this collaborator to complete either UW-Madison human subjects training through CITI or comparable human subjects research training elsewhere.
Question 2.3: If UW-Madison is NOT serving as the IRB of record for collaborating personnel and their own IRB is reviewing, select YES. The IRB approvals from collaborating sites can be uploaded to 2.3.1. If UW-Madison IS serving as the IRB of record for collaborating personnel, then select NO for 2.3.
