Does my research with coded information or specimens require IRB review?

The UW-Madison requires all research with coded private information or coded biological specimens to be submitted to a UW-Madison IRB for review.  Such research can raise ethical issues (e.g., specimen sources) that make the IRB review prudent.  Depending on the specifics of the project, these should be submitted as either an Application for Initial Review or an Exemption Application.
