WiscList Custom - Request Form Guide

This document outlines the process for requesting a new WiscList Custom list.

Getting Started

To start the process for requesting a list, navigate to the WiscList Custom Request Form and log in with your NetID. If you have questions throughout the process, please contact us at wisclist@doit.wisc.edu.

Welcome

There are three ways to set up a WiscList Custom list. Choose the option that most closely matches your needs by clicking the corresponding button at the bottom of the form. At this time it is not possible to mix and match populations, but you may instead request more than one list.

Options

  • Faculty/Staff: Creates a list containing faculty and staff, including student employees. Data is pulled from HRS.
  • Student: Creates a list containing solely students. Data is pulled from ISIS.
  • I will provide my own list of contacts: Creates a list based on a data file that you periodically send to the WiscList servers. These lists require special approval and additional configuration so you will be contacted by the WiscList team after submitted your request. If you need to create a list with a one-time import, use a standard WiscList and see WiscList - Adding Many Members - Import/Export Tools.

When you have made your selection, press Go to begin the request.

Employee Selection Criteria

This page contains options to help you narrow down the population you'd like to select. Note: No fields are required, but you must make a selection from at least one field.

  • Intended Audience: Briefly summarize the faculty/staff population you would like to make up your list. You may type anything you'd like in this field; if we are unable to get the data you're requesting, we'll contact you.
  • Employee Status: Choose employees based on their status with the UW Madison. Unless you specify a choice in this section, we will use employees who are Active and on Leave.
    • Active: Employees who are currently working at the UW.
    • Leave: Employees who are currently on a full leave of absence. This includes faculty with 9-month appointments during the summer months.
    • Terminated: Employees who no longer hold an active or on leave appointment with the UW. Emeritus faculty are typically in this category, for example.
  • Employee Class: Choose employees based on their classification in the HR System. See HR - Empl Class Overview for more information. Note that all employee classes will be used if no selection is made.
  • Special Instructions: If you have special requirements for the list or didn't find the choice you were looking for in the other sections, please let us know in this section.

When you've completed this step, click Next. Jump to the Requestor Information section in this guide.

Student Selection Criteria

This page contains options to help you narrow down the population you'd like to select. Note: No fields are required, but you must make a selection from at least one field.

  • Academic Year: Choose any combination of academic years if you would like to narrow the scope of the list. Students who are enrolled in undergraduate studies but have been at the UW longer than 4 years are considered Seniors for the purposes of this form. Note that all academic years will be used if no selection is made.
  • Programs / Majors: Choose any combination of majors, certificates and programs in which students are enrolled. You may type anything you'd like in this field; if we are unable to get the data you're requesting, we'll contact you.
  • Degree Type: Choose any combination of degree types if you would like to narrow the scope of the list. Note that all types will be used if no selection is made.
  • Gender: You may choose a gender to narrow the scope of your list if desired. Students who do not identify as male or female may only be selected using the all option. Note that all genders will be used if no selection is made.
  • Enrolled Courses: Choose any combination of courses in which students are enrolled. This may include courses by name, number, topic, college, etc. You may type anything you'd like in this field; if we are unable to get the data you're requesting, we'll contact you.
  • Special Instructions: If you have special requirements for the list or didn't find the choice you were looking for in the other sections, please let us know in this section. Some examples might include a student's citizenship or targeted minority status. As with the entire process, you must have a valid business need for using this data as determined by the data custodians.

When you've completed this step, click Next. Jump to the Requestor Information section in this guide.

Requestor Information

We need to know a little about you and what your intended usage of this list to get your request approved. Please complete all sections on this page, with special care to explain the purpose of this WiscList Custom request. Some information is pre-populated for you.

When you've completed this step, click Submit to send us your request. You will receive an email confirmation once the request has been received.
