UW Digital ID - Customer - Account Locked Due to Incorrect Contingency Question Answers While Requesting a Temporary Static One Time Password

This document summarizes what to do should you lock your account after answering your contingency questions incorrectly while trying to generate a temporary static one time password.

When you have entered an incorrect response to the contingency access questions 3 times you will see the following message: 
"You have been locked out of contingency access due to too many incorrect security question answers."

1. Contact your Local Registration Authority (LRA) and inform them you have locked your contingency access

2. To find out who your LRA is click on the Your Campus LRA link at the bottom of the page within the UW Digital ID site

3. To verify your identity you must present at least one form of government issued photo ID to your LRA
Acceptable forms of Identification: UW ID Card, Drivers License, Passport, US Military ID Card

4. After you have verified your identity, the LRA can then unlock your contingency access

Important Note: your OTP device will still work even though contingency access for a temporary static OTP is locked.
