Faculty and Academic Staff must report leave monthly. This document provides information on how to report leave.

Leave Reporting

Once a month, faculty will receive a yellow leave reporting form in your mail box.The form is for you to report your leave usage for the prior month. Complete, sign, date and return the form to Carrie Hill. If you misplace your form, you can print another copy by logging in to “MyUW” and clicking on the “Work Record” tab. Failure to complete and submit leave reports can result in a reduction of sick leave accrual.

Sick Leave Reporting

Full time employees report sick leave in half and full days (4 hours and 8 hours) according to the following chart:

If you miss:

Report leave as:

< 2 hours

0 hours

2-6 hours

4 hours

>6 hours

8 hours
