Audio Recordings/Video Recordings/Photographs

If audio recordings, video recordings, or photographs are being taken of your participants and used for research purposes, indicate this on the application. 

Other considerations:

- If identifying photos or videos of participants will be published, it is likely the IRB will require a photo release be signed, in addition to consent forms.

- If recordings or photographs of minors are being included, these cannot be retained indefinitely or minor participants will need to be re-consented upon turning 18 years old.
