Acuity One45 - Form Status Report
This document outlines how to access the curriculum calendar for faculty and admin within Acuity One45 at SMPH.
Outstanding Forms
To view outstanding forms with student emails:
- Navigate to Reports > Report Center > Form Status Report
- Select the date range for which the evaluations were sent out.
- Select the form(s) you want to look at.
- Select Run Report
- Under 'Filter' add three filters to filter out the form statuses of "completed", "closed", and "deleted". You will have to manually add these, but see the screenshot below for what should be added.
- Click "Ok"
- Click on 'Show/Hide Columns" and click on the checkbox for 'Evaluator Email'.
- Click "Ok"
- From here, you can export to excel and access all the students' emails who have not completed the form.

Used Saved Report
You can use a saved report to view the same information as above, but just edit a few details.
To run a saved report:
- Navigate to Reports > Report Center > Form Status Report
- Click on the saved report on the left hand side (ex. outstanding forms with email).
- Edit the dates and form you wish to view.
- Once you run the report, all filters and columns will be applied, so you can export straight to excel or further edit data in Acuity One45 prior to exporting.
