Acuity One45 - Phase 3 Form Status Queries

This document describes how to access saved report that query department/location/activity's completed and incomplete forms.

Phase 3 saved queries provides filtered reports on incomplete and completed forms (assessments and evaluations) by location, department, and activities such as the Acting Ambulatory Internship, and Inpatient Acting Internship. 

  1. Go to Reports on the top menu, then Reports Center on the left blue menu. 
  2. Click Form Status Report.

  3. The screen will reload to the Form Status report. 
  4. On the left side of the screen there will be a list of reports. Each query variable has two reports, a report of Completed forms, and a report of Incomplete forms. 
    The brief description may help determine which report will be of most use.

  5. Click on the report name - this will start to apply the filter by automatically selecting the forms.

  6. Select the date period to report on – This date range refers to the rotation dates associated with the forms. 
    NOTE: Selecting dates will significantly help reduce the time to pull the report. Including the entire date range will greatly increase the report wait time.

  7. After selecting the date, click Run Report.

  9. After the report has finished loading, a table will appear with the query results.

    1. Form Type: 
      1. Distribution: A distribution that was sent to the coordinator to send to the preceptor.
      2. Contributing: A form that was distributed to the preceptor to complete, that requires a Head Evaluator form. 
      3. Head: A head evaluator form.
      4. Normal: A form that was distributed to the preceptor to complete, and does require a Head Evaluator Form. 
      5. Single Send: A form that was distributed through the Bulk/Single Send Function.
    2. Form Name: The name of the assessment form.
    3. Evaluator Name: The first and last name of the preceptor and/or head evaluator who as assigned the assessment form.
    4. Target Name: The first and last name of the student in which the form was completed about.
    5. Target Student/Employee/faculty number: The student id number.
    6. Group Name: This should always read Phase 3.
    7. Activity: The name of the course option.
    8. Activity Abbreviation: The name of the course option and includes course number.
    9. Form Status: The status of the form
      1. Sent: The form was distributed, but not yet completed
      2. Closed: The form was not completed before the due date
      3. Not Distributed: The form is currently in the To Do box of the form distributor
      4. Completed: The form was completed
    10. Evaluation Start Date: The start date of the assessment
    11. Evaluation End Date: The end date of the assessment
    12. Evaluation Sent Date: The date the assessment form was sent to the preceptor and/or Head Evaluator to complete
    13. Evaluation Completed Date: The date in which the assessment was completed. 
    14. Days to Completion: The number of days the preceptor took to complete the form from the date received.
  10. Use the search bar to find a specific piece of information within the query.

  11. The report can be downloaded into excel by clicking the blue excel button
