REDCap: Service Agreement
This Service Agreement must be completed for the initial move of a research project from Development to Production status and annually thereafter until data entry and edits are completed.
Click to download a copy of the SMPH REDCap Service Agreement. Once completed, return the signed agreement to REDCap support staff at redcap@ictr.wisc.edu.
Please note: You must include your Project ID or PID number on the agreement along with the REDCap project title. This unique project ID is the easiest way to identify your project. To locate your PID follow the instructions here: How to find your Project ID (PID).
