To obtain passport, appear in person at passport service office with:
1) a certified copy of birth certificate (hospital issued birth certificates not accepted), Original Naturalization Certificate, or an old passport;
2) 2 identical recent color passport photos (photo service offered at Milwaukee St. location only, for $15.)
3) a valid ID that has your physical description and a picture on it; and a valid driver's license ages over 16 or a WI ID card ages under 16
4) ***Two separate payments (Standard Application):
Under age 16*: $80 check or money order payable to US Dept of State AND $25 cash, check, or money order to Postmaster or Clerk of Courts (depending on location)
Age 16 and over*: $110 check or money order payable to US Dept of State AND $25 cash, check, or money order to Postmaster or Clerk of Courts (depending on location)

(Takes approximately 6-7 weeks to process)

Regular passports are valid for 10 years.

For emergency passport, take all information (previously listed requirements, copy of air itinerary, and it must be less than 2 weeks from departure) to the Chicago Passport Agency, located at the Kluczynski Federal Building, 230 S. Dearborn Street, 18th Floor, Chicago, IL 60604. You need to schedule an appointment in advance by calling (877) 487-2778. There is an additional fee of $60 for expedited service.
