Medical Record Number - Field "MRNo" (WRAP)

This KB Article describes the "MRNo" field found in the "Everyone" table of the WRAP Database
The medical record number can be found in OpenClinica (under Baseline; Enrollment.) The number is assigned by the UW Hospital Registrar to identify patients. The Clinical Research Unit (CRU) requires this number in order to reserve study visit appointments. The hospital registrar keeps a permanent record of this number for each patient and it is associated with their HealthLink (HL) record which includes the patient's demographic and contact information.
If the participant receives their medical care through the UWHC system, then accessing this MR# will allow viewing of that participants full medical history with UWHC.  If the participant does not receive their care through UWHC, the only medical records associated with this number is the information that the CRU records during the visit (i.e. vitals) or clinician notes from a lumbar puncture visit. 


There is also an EPIC (Electronic Medical Record) Number in HealthLink. A participant can be looked up in HL with either number. WRAP does not use or store this number.

Confusingly, the UW Health Lab includes a "MR#" on the faxed blood lab results.  Because WRAP participant's labs are considered anonymized, this number is actually a requisition number not an MR#.  Every lab result form for a WRAP participant has different requisition number.  This number is not connected to the participants medical record currently.  When WRAP is able to use HealthLink, the lab results will be resulted under the participant's MR# in HL.
