The Ovary removal questions had responses where the content was identical, but order of responses different between Entry Assessment (New) / 2nd Wave (Old) 
and Subsequent Wave (New) and Waves 3 onward (Old).  Add new variable for New Entry Assessment and Old Visit 2 data points.  

All of the responses were originally placed in the "OvaOut" field of the "WomensQuestions" table.  This caused some confusion. 

The values of the "OvaOut" field have now been also placed into the "OvaOut_InitialResponseSet" or "OvaOut_SubsqResponseSet" fields.

Note that for more recent data entry, the "OvaOut" field may be blank, but there may be a value in the "OvaOut_InitialResponseSet" or
"OvaOut_SubsqResponseSet" fields.

           Note: The phrase "(Ovaries are the oval shaped egg-producing 
                     reproductive organ in females)" is present in Visit 1
                     questions.

   5. Yes, part of an ovary was taken out
