COVID Screener for UW Hospital (and CRU)

Every participant entering UW Hospital must go through a COVID screener. Coordinators should call participants 1-3 days in advance of their study visit to ask the COVID Screener Questions.

Please call your participants 1-2 days in advance to share our visitor policy and more importantly, screen them for COVID-19 (regardless of their vaccination status):

  • Any symptoms of COVID-19 infection (e.g. Fever ≥100°F, Chills, Cough, Sore throat, Shortness of breath/chest tightness, Loss of taste or smell, Runny nose, Nasal congestion, Headache, Severe fatigue/exhaustion, Muscle pain, Nausea/Vomiting, or Diarrhea)
  • Positive COVID-19 test in the last 20 days
  • Pending COVID-19 test obtained because of symptoms associated with COVID-19
  • Exposed to a person with a positive COVID-19 disease within the past 14 days regardless of vaccination status or masking

If any of the above items are true, their CRU visit should be rescheduled. Please contact the CRU if you have any questions or concerns with delaying the study visit.
