WRAP Scheduling Script - Telephone Cognitive Testing Visit

The script pertains to subjects enrolled in the WRAP study protocol #2016-0634 who are being invited to complete a Telephone Cognitive Testing visit in lieu of in-person testing.

WRAP SCHEDULING SCRIPT

TELEPHONE COGNITIVE TESTING VISIT

Schedule visit 1-4 weeks from call
Plan on 1 to 1.5 hours for Telephone Cognitive Visit
Consent, questionnaire review and cognitive testing
Medical exam and biomarker visits MAY be scheduled separately

 

For Subsequent Visits

  1. The consent form you signed at your last visit included follow-up over the phone. At the beginning of the visit we will ask for your verbal consent to record portions of the visit.

 

  1. We will mail you a packet with the following:

 

    1. Questionnaires: Please return the completed questionnaires to WRAP in the enclosed postage-paid envelope at least one week prior to your visit; this information is important for your visit and we will ask to do a shorter questionnaire over the phone if we have not received it in time.

 

      1. Complete the questionnaire section of your medications and over the counter drugs. Please use your bottles to complete this section. If available, please send us a list of your medications from your medical record or photographs of your medication bottles when you return the questionnaires.

      2. The questionnaires will have you designate someone who knows you well that we can send questionnaires to about your memory or thinking. We will confirm with you during the telephone interview whether the person you used last time is still applicable.

 

    1. An information sheet summarizing what will be done during the telephone interview, confirming the date and time of your appointment, and dates by with to return your questionnaires.

 

  1. The telephone interview will include:

Consent discussion
Questionnaire review
Cognitive testing

Options for medical exams and biomarker visits

For Wave 1 Visits

 

  1. We will mail you a packet with information about your visit. It will include one copy  of the consent form. We will do a 15 minute call after we mail your packet to go through the consenting process. We will go through the consent form and, if you would like to continue with the study,  we can keep your scheduled visit.  If you come in for any in-person study components, you will sign the consent at that time

 

  1. The Packet will also include:

 

    1. Questionnaires: Please return them to WRAP in the enclosed postage-paid envelope at least one week prior to your visit; this information is important for your visit and we will ask to do a shorter questionnaire over the phone if we have not received it in advance. The questionnaires take about 45 minutes to complete
      1. The questionnaires ask about medications and over the counter drugs you take. Please use your bottles to complete this section. If available, please send us a list of your medications from your medical record or photographs of your medication bottles when you return the questionnaires.
      2. The questionnaires will have you designate someone who knows you well that we can send questionnaires to about your memory or thinking. This person someone you spend time with frequently, who can provide some general information about you and describe how your memory is for things in your daily life. We will confirm the name you provide during the telephone interview.
    2. An information sheet: summarizing what will be done during the telephone interview, confirming the date and time of your appointment, and dates by with to return your questionnaires and consent form.
  1. The telephone interview will include:

Include the following for TCog visits in Madison and Milwaukee Starting 7/1/24 (Refer to Advarra SOP for more info:Advarra SOP)

    • The University of Wisconsin recently updated the system used to provide compensation to research study participants. The Advarra Participant Payment program will give you the choice to select from a Visa debit card, direct deposit, or check at your next study visit. Your pre-visit packet will include details about each of these choices and your study coordinator will be able to answer your questions and support your set-up. Would you like additional information at this time?

    • For Cash Participants:  The University of Wisconsin recently updated the system used to provide compensation to research study participants. The Advarra Participant Payment program will give you the choice to select from a Visa debit card, direct deposit, or check. I understand you may (have received/were offered) cash payments and may have questions about the new payment options.  Your pre-visit packet will include details about each of these choices and your study coordinator will be able to answer your questions and support your set-up. Would you like additional information at this time?
