Hospitals and Clinics Under the Purview of the UW

No additional approval beyond IRB approval is needed for UW-Madison employees to conduct research in the facilities that follow (except as noted). Additionally, these hospitals and clinics are operated by entities that have contracted with UW-Madison for UW-Madison’s IRBs to review the research activities of their employees (i.e., we have an IRB Authorization Agreement to act as IRB of record for these facilities that do not have their own IRB).

Additional approvals are needed for UW-Madison employees to conduct research within the following hospitals and clinics affiliated with UW Health (e.g., joint venture, joint operating agreement, subsidiary).Additionally, UW-Madison IRBs are not the IRB of record or the only IRB of record for these facilities when their employees are engaged in UW-Madison research

NOTE:

  • UW-Madison criteria for ceding to or serving for (e.g. Appendix A+B) do not apply to Meriter given the close association between the two. Which IRB is the most appropriate to serve will need to be assessed on a case-by-case basis.
    • For studies involving only chart reviews, IRB applications must be submitted to both IRBs. This is because Meriter uses the IRB process as a gatekeeper for HIPAA security review.
  • UW-Madison will serve for Swedish American, but not cede.

Entities for Which UW-Madison is Not the Reviewing IRB OR Not the Only IRB of Record

Hospitals:

  1. UW Health Rehabilitation Hospital (partnership with UnityPoint Health – Meriter and Kindred Healthcare)
  2. UnityPoint Health – Meriter (Meriter and UW use SMART IRB)
  3. Swedish American Health System (SAH), d/b/a "UW Health Northern Illinois"
    1. Includes Swedish American Hospitals and other family practice and specialty clinics in northern Illinois.

Under the UW, UWH and SAH Data Sharing Agreement, UW personnel can request access to SAH medical records for research purposes through ICTR’s CHI2/CRDS. If a UW researcher will be recruiting participants from a SAH facility or collecting data from participants within a SAH facility, additional SAH approval is needed.

Clinics:

  1. UW Cancer Center Johnson Creek (joint collaboration between UW Health, Fort Healthcare, and UW Health Partners Watertown Regional Medical Center)
    1. Johnson Creek employees engaged in research are covered by an IAA with UW
  2. Wingra Family Medicine Clinic (owned and operated by ACHC)
    1. Approval to use records from the clinic or to recruit patients within the clinic is obtained through ICTR's CHI2/CRDS
  3. Chartwell (separate entity that is affiliated with UWHC; home infusion service, so research is highly unlikely.)
