Computer Sciences - Human Resources - Minor Hires and Volunteers
Information needed when a minor student is hired or is a volunteer.
Here are three attachments needed:
1.) Checklist for Labs to have Minor Volunteers (.doc)
This checklist outlines the approval process by the Department Chair, laboratory safety requirements, description of the work and the parent letter explanation.
This checklist also notes a criminal background check will be required by all individuals who work in the lab. CBCs are currently done through L&S HR.
This checklist also includes a link to a Mandated Reporter Online Training module, the completion of which will be required by all individuals who work in the lab.

2.) Consent and Release Form (.doc)
This waiver/release example form includes a photo release. This should be tailored to the activities and hazards in the laboratory.
3.) Draft Sample Letter (.docx)
This letter should be formatted with current information which will include the activities and hazards in the laboratory. This letter will be placed on Department letterhead and signed by the Department Chair.

In addition, the UW Director of Environment/Health/Safety will need review your laboratory and safety plan for this activity and your Department Chair will also need to approve this activity.As of May 2017, the Department must retain the copies of these documents. Documents are only sent to the UW Office of Risk Management in the event of an accident or injury. The documents need to be retained for indefinitely.
