OHRP Audit Resolution

The Office of Human Research Protections (OHRP) has concluded its not-for-cause audit of the University's human research subjects protection system. Two minor findings were noted in the close out report. We are pleased that our program continues to meet the high standards set by OHRP and other agencies.

IRB Meeting Attendance
In order to keep IRB meetings running as smoothly and efficiently as possible, please keep the following in mind:
  • If you confirmed your meeting attendance, but cannot make it due to an unexpected conflict, you will be expected to complete your reviews in most cases. Please inform the IRB of your absence and forward your completed reviews to the staff reviewer assigned to the study with a copy to the IRB Administrator, Gretchen Anding ( for the Minimal Risk IRB and Nichelle Cobb for the Health Sciences IRB ( If you cannot complete your reviews, contact the IRB Administrator as soon as possible so that she can reassign to another committee member if possible.
  • If you need to leave an IRB meeting early, please inform the Chair or IRB Office as soon as possible of when you will be departing.  This is important in terms of anticipating any issues with quorum or VA representation.
  • If you are assigned as a primary or secondary reviewer for an initial review, please review Guidance for UW Madison Health Sciences IRB & Minimal Risk IRB Primary Reviewers for Summarizing Initial Review Applications when preparing materials for the meeting.

2013 IRB Meeting Schedules and Attendance Confirmations
The HS IRBs Office has begun to confirm attendance for the 2013 MR IRB and HS IRB meetings among each reviewer group.  We are requesting that all IRB members review and reply to requests for confirmation sent from  Your prompt reply is appreciated in creating the 2013 MR and HS IRB meeting schedules and confirmation of attendance prior to the end of the calendar year.   We would like to thank our HS and MR IRB members for the past and continued service to the HS IRBs.

Spotlight on HRPP Policy: IRB Member Conflict of Interest
The UW-Madison has a policy that defines when IRB members are considered to have a conflict of interest and thus cannot participate in the review of a research study. Any IRB member with a conflict of interest in a research protocol under review by the IRB must disclose the conflict of interest to the IRB Chair and leave the room during the discussion of the protocol and the related vote, except if the member is providing information at the IRB’s request. IRB members have a conflict of interest with respect to a protocol when:
  • They or an immediate family member has a professional interest as a principal investigator or co-investigator in the protocol, or
  • They or an immediate family member of an IRB member has an interest that is related to the research and that meets or exceeds certain financial thresholds or has certain relationships with a business entity involved in the research.
  • They have a proprietary interest of any value including, but not limited to, a patent, trademark, copyright or licensing agreement except those managed by the Wisconsin Alumni Research Foundation (WARF).
  • They or an immediate family member have a personal relationship that may cause bias or create the appearance of bias by the member or consultant in the review of the protocol.
