UW-Madison's use of DocuSign is licensed by the Wisconsin Department of Administration's DocuSign agreement.

Anyone with an @wisc.edu account can request access to Docusign. Please note that these account requests may take up to 48hrs to process.
Note: In order to access the Google Form, you must be signed into your UW Google account/G-Suite. Users trying to access the form without an account or with a personal account will receive a permission error. If you already have a Docusign account, you can sign in using the blue button below:

For General Users

Other Docusign Products
Docusign may mention products that offer search capabilities. These are NOT included and would require an additional subscription at additional cost:
- Connect - real-time push.
- Retrieve - pulls to your server on your time schedule; allows for integration into other systems (info look up, for example).
- Seal - Search capability; details of documents being signed; can use non-Docusign documents with these products as well; two potential pieces:
- Total Search - Indexes documents' info after putting it into a specific repository.
- Intelligent Insight - provides better results and better insights.
