The Telephone Activity Report has moved to its new website.

In order to place telecom orders and to access the Telephone Activity Reports, you must be authorized. This document explains how to obtain and remove the authorization.

Access Requests

Individual departments determine who has access to place telephone orders and receive access to the Telephone Activity Reports for specific general ledger numbers. Changes to access must be made on an individual basis using the Telephone Authorization Request Form. Access requires a UW-Madison NetID. Please follow these instruction for activated your NetID: NetID - Activating Your Account

Reports will be available at the website for one complete fiscal year plus the current fiscal year to date. The earliest report available will be for the May 2020 billing period.

Terminating Access

If a person leaves a job, the removal of the user can be requested using the Telephone Authorization Request Form. It is the department's responsibility to ensure that someone is receiving access to reports for each of your general ledger numbers. DoIT Voice Services has no way of knowing if reports are not being accessed/viewed unless informed by campus departments.
