Qualtrics Survey Hosting - Service Description and Support Resources
This document provides general information on accessing the Qualtrics Survey Hosting Service and explains how users can get help.
Qualtrics Survey Hosting Service (SHS) is a web-based survey service available to all UW-Madison faculty, staff and students. Qualtrics SHS allows users to create surveys, collect and store data, and produce reports.
UW-Madison Students and Staff can access the system at http://survey.wisc.edu. This URL provides links for general information about the service, best practices and logging in.
All current UW-Madison staff and students are eligible to use the system. No independent account creation or subscription is required. There are, however, some directory data requirements. For details, please see: Qualtrics Survey Hosting Service - What is required to use the system?
The URL for access to the service is http://survey.wisc.edu.
Getting Help with Qualtrics Survey
Users who are experiencing problems or receiving error messages during the login process can contact the DoIT Help Desk for assistance. See Get Help from DoIT for details.
Users who have questions about using the Qualtrics Survey Hosting Service or who experience problems using the application can contact Qualtrics directly for assistance. Qualtrics offers support via phone, email and online documentation:
- Phone: 1-800-340-9194 (Monday-Friday 8 a.m.-7 p.m.)

Users that would like to transfer ownership of surveys, create or change groups of Qualtrics users, or who otherwise need assistance from a UW-Madison Brand Qualtrics Admin can email email@example.com to facilitate these requests.
