Ordering telephone service (Cisco VoIP, Cellular/Mobile, Off-campus Landlines)
Ordering telephone service requires you to be an authorized user:
Cisco VoIP
Cisco VoIP service can be ordered by using these forms.
The majority of campus uses Cisco VoIP for their landline services. The phones run on the data network. Users either use a Cisco-branded phone or Cisco Jabber software on their computer for phone service.
Cellular/Mobile Service
Cellular services can be ordered or canceled using this form.
For other questions and changes related to Cellular service (Example: Request to change a cell line to a different assigned user..., Misc question etc), please use this form.
Please note equipment cannot be ordered without service. Smartphones require either a smartphone or data plan.

Off-campus Landline Service
There are also non-Cisco phones that are located off-campus. These off-campus numbers start with something other than 608-26X or 608-890. Off-campus telephone services can be ordered by using this form.If you are looking for fax service, please see CloudFax - Getting Started
Toll-free Service
A toll-free number allows callers to contact a phone number outside their local calling area without incurring long-distance fee from their telephone service provider. However, most telephone plans no longer incur additional fees for calling outside their local calling area. Toll-free numbers point to an existing local number. You will not be able to call out from a toll-free number. To request a toll-free number to route to one of your existing phone numbers, use this form. There will be no charge for this service for UW-Madison departments.
Centrex
DoIT is not longer adding Centrex service. Please request Cisco VoIP service.
